Registered Nurses (RN)

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community in order to plan and direct services to meet the needs of individuals and families within their homes and communities.
Qualifications
  • Licensure to practice professional nursing in the state of Texas
  • Maintains a current CPR certification
  • Minimum of 1 (one year) experience in home healthcare
  • Current TB or X-Ray Test

Licensed Vocational Nurse (LVN)

The Licensed Practical/Vocational Nurses are responsible for providing direct patient care under the supervision of a registered nurse. They have responsibilities to follow-up the plan of care, providing treatments, and working collaboratively with the members of the team in order to help meet positive patient care outcomes and target goals.
Qualifications
  • Provides direct patient care as defined in Texas Nurse Practice Act
  • Implements plan of care initiated by the Registered Nurse
  • Provides accurate and timely documentation consistent with the Plan of care
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and Plan of care
  • Minimum of 1 (one year) home health experience
  • Licensure to practice professional nursing in the state of Texas
  • Maintains a current CPR certification
  • Current TB or X-Ray Test

Home Health Aide (HHA)

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed. Responsibilities of the home health aide include, but are not limited to the following:

Bath; oral Hygiene; shampoos; changing Bed linen; assisting patients with dressing and undressing; skin care to prevent breakdown; and assisting the patient with toileting activities.

Qualifications
  • Minimum of 1 (one year) home health experience
  • Certified with the State of Texas
  • Maintains a current CPR certification
  • Current TB or X-Ray Test

Physical Therapist (PT)

The physical therapist contracted or employed through the agency is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.
Qualifications
  • Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization
  • Licensed to practice as physical therapist within the State of Texas
  • Two years of appropriate experience as a physical therapist

Physical Therapy Assistant (PTA)

The physical therapist contracted or employed through the agency is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Occupational Therapist (OT)

The Occupational Therapist contracted or employed through the agency is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement.

Speech Pathologist

The Speech-language pathologistor is the specialist with the role to assess, diagnose, treat and help prevent speech, language, cognitive-communication, voice, swallowing, fluency and other related disorders.
